Care Manager RN Job DescriptionOverviewSwedish Health Services is seeking a skilled Care Manager RN to coordinate the care and service of selected patient populations through the acute care episode, across the continuum.Main Responsibilities

• Collaborate with inter-disciplinary teams to improve patient care through effective utilization and monitoring of healthcare resources
• Assume a leadership role to achieve desired clinical, financial, and resource outcomesKey Qualifications
  • Bachelor's Degree in Nursing or related field
  • Washington Registered Nurse License upon hire
