• NIH Stroke Certification required for ER
• Medication Test/Pharmacology Competency Exam required
• Competency Exam required periodically and for specific units

    • Additional Details
    • ACLS: American Heart Association Only (Urgent care, Periop). Copy of the front and signed back required
    • BLS: American Heart Association only. Copy of the front and signed back must be uploaded
    • PALS: American Heart Association Only (Urgent care, Periop). Copy of the front and signed back required
    • AHA only - Required for PACU, Acute Care, CCU, and ER
    • NRP: American Heart Association Only (NICU)
    • WA License Verification: must be within 30 days of start and/or submittal date
    • Washington State License
    • Required for ER. Can be from any issuing organization
    • Must 80% unless the Angoff score is higher than 80%. If the Angoff score is higher than 80%, the traveler must meet or exceed that score
    • Passing score of 80% required; required every 3 years
    • Unit specific competency exam required
    • WATCH: to be completed prior to start. This is run by QM
    • FDI - Manager will reach out to traveler directly
    • Badge Photo is required prior to start
    • Sample FDI - FOR QM USE
    • REQUIRE when an RN or LVN is using a Multi State license from any state other than WA and will be working in state of WA. Prior to working in the state of WA with a compact license, candidate must complete a 6 hour suicide prevention training course from an approved course list provided by the WA BON
    • Certification of completion required
    • RN's & LPNs working with an out of state Compact License in Washington are required to complete the WA - Multi State Licensed - Demographic Attestation prior to starting their assignment
